Job Summary
The Senior Community Health Advisor (SCHA) educates potential PACE participants about the PACE program, and assists eligible Seniors, their families and/or designated caregivers through the enrollment process.
The SCHA is an ambassador in the community and works alongside the Business Development Department to support a team of Community Health Advisors to promote the PACE program within target communities.
They also train Community Health Advisors to provide education and assist in introducing PACE services to eligible seniors and set team goals.
Essential Job Functions
Duties include, but are not limited to:
Work with the Business Development Department and Community Health Advisors to identify target communities.
Educate community members and/or their families/caregivers.
Coordinate tours and presentations at the PACE facility and provide referral sources.
Develop relationships within target communities and educate them on the PACE program.
Participate in community events and PACE-sponsored events providing health and social services education to community members.
Assist in the setup of health education events and PACE information booths.
Participate in mandatory and ongoing training and skill-building activities to effectively achieve community outreach and program goals.
Assist with setting goals and supporting team performance outcomes.
Conduct preliminary enrollment questionnaires to determine eligibility for PACE program services.
Assist in the recruitment of staff to support new participants including but not limited to Community Health Advisors, Promotoras, and Interpreting.
Conduct training for new Community Health Advisors and other members of the Business Development and Outreach team.
Provide support to Community Health Advisors, per the assigned location.
Report and provide recommendations to management based on knowledge coming from the field.
Guide potential participants and/or preferred caregivers through the intake and enrollment process.
Identify and develop relationships with community-based providers (CBP) in order to provide best solutions for the language challenges facing prospective participants, while creating opportunities for eligible referrals to the PACE program.
Build strong relationships with CBP and referred patients, providing education on PACE program services and assisting them through the intake and enrollment process.
Actively monitor referred participants’ satisfaction with their care plan and assist when necessary.
Translate, as needed, for PACE participants, as well as care providers and support staff.
Track enrollment activity as directed by the Marketing Manager and the VP of Business Development.
Handle and respond to requests and queries as necessary.
Assisting with other administrative duties as directed, including setup of presentation materials.
Accept assigned duties in a cooperative manner; and perform all other related duties as assigned.
Be flexible in schedule of hours worked, evenings and weekends may be required.
May require use of personal vehicle.
Knowledge, Skills, and Abilities
Second language strongly preferred.
Familiarity with the healthcare delivery system through education, training, or experience.
Excellent communication skills, both orally and in writing.
Confidentiality is a must.
Must work in compliance with all HIPAA requirements.
Ability to quickly learn department policies, procedures, goals, and services.
Ability to change priorities regularly.
Strong customer service and grievance resolution skills.

Ability to lift up to 35 pounds occasionally, 15 pounds frequently, and 7 pounds constantly; required to obtain assistance of another qualified employee when attempting to lift or transfer objects over 25 pounds.
Requires constant hand grasp and finger dexterity; frequent sitting, standing, walking and repetitive leg and arm movements, occasional bending, reaching forward and overhead; squatting and kneeling.
Ability to communicate verbally with an excellent comprehension of the English language.
Work is generally performed in an indoor, well-lighted, well-ventilated, heated, and air-conditioned environment.
Requirements:
Experience
Minimum of one (1) year of demonstrated successful experience in health care.
Minimum of one (1) year of documented experience working with a frail or elderly population, through education or experience.
Proven experience as a customer service, sales, education, training, and/or marketing representative.
Education and Certification
Must complete the PACE Marketing Certification under the supervision of the Marketing Manager.
Bachelor’s Degree strongly preferred.
High School Diploma required.
Valid California Driver’s license.
Is medically cleared for communicable diseases and has all immunizations up-to-date before engaging in direct participant contact.
