Families Together of Orange County (FTOC) is a rapidly growing Federally Qualified Health Center (look alike) located in Tustin, CA looking for a full-time experienced Case Manager.

Job Overview: Under the supervision of the Social Services Manager.
The Case Manager's responsibilities include conducting intakes with prospective clients, providing crisis management for clients and making linkages for interventions as appropriate; providing ongoing support and assistance to current clients as needed; providing support at FTOC facility; maintaining current knowledge of community resources and public benefit program and interfacing with community organizations.
Essential responsibilities:
Interact with a diverse client demographic that includes factors such as at risk of homelessness, mental health needs, domestic violence and other biopsychosocial factors
Demonstrate sensitivity toward all participants, regardless of race, creed, sexual orientation, present living condition, needs expressed or nationality and documentation status
Provide intake, assessment and service planning of new clients
Linked participants to FTOC Medical, Dental, Behavioral Health and other services
Educate and refer clients to community resources and public benefits; follow up with clients and agencies as appropriate to document use/success of referral
Provides case management and oversight/coordination at FTOC
Attend community collaborative meetings on weekly basis to remain current on resources and opportunities available to participants and the community
Participate in staff meetings and trainings as directed by supervisor
Maintain confidentiality of participant interactions in keeping with the FTOC policy
Learn and demonstrates knowledge of agency and program policies and procedures
At all times represent the organization in a professional manner
Other duties as assigned.
Administrative duties:
Follow up procedures as outlined by Families Together of OC as well as partner agency
Ensure compliance with all agency policies/grant requirements
Practice effective time management and communication skills
Complete and update client files in a neat, orderly, and timely manner
Maintain records of submitted check requests, current status, and any other information needed to assist with reporting to funder
Protect the confidentiality of all client related records
Maintain current awareness of community-based resources and public benefits available to clients including Orange County Social Services, Social Security and other non-profit services agencies.
Additional Qualifications
Case management experience required including working with homeless and at risk of homelessness population; person in crisis
Education: Bachelor’s Degree or must have at 2 years of full-time related work experience
Experience working in Orange County and knowledge of social service providers
Computer/office equipment skills: Must have proficiency with Microsoft office, particularly Word, outlook and Excel.
Must be familiar with utilizing client database software- HMIS
Excellent communication skills, both written and verbal
Other skills: Must work well in multicultural teams and diverse constituencies; must be self-motivated and highly organized with the ability to effectively work under pressure, use independent judgment and produce a quality of work product
Spanish Required
Available to work evenings and weekends as needed
Valid CA Driver License
